Walk-In
Registration
Complete program registration
in person anytime Arrowhead Community Center or Heritage
Crossing Field House is open
1. Payment
for all activities must be made at time of registration or
registration is not complete or official.
A.
Payments by check, cash or credit card.
B.
Returned checks will result in a $20 fee.
2. Include
copies of any additional registration items needed as specified in
program write-up, such as birth certificates.
A. The
Channahon Park District may make a copy of the item(s) to keep
on file.
B.
Registrations without these items may not be processed and may
be returned.

Mail-In
Registration
1. Include
a separate check, or include VISA/Mastercard payment information for the appropriate amount, for each class you
are registering for.
A. In
the event the class is full, that check will be returned to you.
2. Do not
send originals of birth certificates or other additional forms
requested for registration, please mail a copy.
3. All
registrations received are processed in random order, beginning
the first day of walk-in registration.
4. The
Channahon Park District is not responsible for lost, stolen,
damaged or late mail.
5. If you
wish to have a receipt mailed to you, please include a
self-addressed stamped envelope.
NOTE: You
may not register by mail for Fitness Center memberships, pool
passes, swim lessons, Fun-N-Learn, facility rentals, golf or
skating passes.

Fax-In
Registration
1. Use
your credit card and fax in your program registration 24 hours a
day! Include type of card, card number, and expiration date
on form.
A. In the event the class is full,
you will be contacted and will
not be charged.
2. Fax completed form to
815-467-5677.
Please note
that all fax-in registrations can only be allowed when accompanied
by a Visa/Mastercard payment.
NOTE: You may not register by
mail for Fitness Center memberships, pool
passes, swim lessons, Fun-N-Learn, facility rentals, golf or
skating passes. |
